Ratthew's Key and the Hidden Safe

Treat Ratthew as a normal patient across roughly five visits - never reject him with the shutter. On his final visit he tells the story of his missing twin sister Rattina and leaves behind Ratthew's Key. Use it on the hidden safe behind the cat painting in the first unlocked corridor to reveal major lore about the Animal Corporation.

Barney's Second Coffee Machine

Complete all five of Barney's visits without refusing or delaying him (coffee, the "Do Not Open" briefcase, a photo, and the scalpel from Room 8). When Officer Duckman arrives with the wanted poster, stay silent for ~30 seconds to hide Barney. He rewards you with a second coffee machine, which is one of the strongest permanent upgrades in the game.

Liz and the UV Camera

Lizarrived with the Harlow's Favorite update and runs the game's biggest side story. During Shifts 11-12 she lends you a UV camera to gather evidence about the anomalies. If you waste it, she never comes back. The UV camera also interacts with the Stalker in a surprising way - a photo of the Stalker burns instantly, and a UV scan comes back completely black.

The Stalker's Photo Trick

Since the Harlow's Favorite update, the Stalker can appear as a check-in patient with a messy, messed-up application form. Taking its photo makes it disappear - an easy way to clear it without losing sanity.

The Bed Monster's Favorite Snack

Instead of risking the red zone under a bed, hold maple syrup, an organ, or a transplant and approach - the Bed Monster takes the item and leaves. Coffee also blows out the candles during anomaly rituals without being consumed.

Slimebert & the Hazmat Event

From Shift 11 to 12, the hospital is flooded with green slime and Slimebert roams the halls. You cannot defeat it, but a fire extinguisher slows it down. Survive the event and Sam, a calico cat in a hazmat suit, joins the hospital to clean up slime whenever it reappears.

Journal & Achievement Coins

The Journal (added in the Hazmat update) tracks every anomaly and NPC you have encountered and comes with a new achievement set. Every completed achievement pays out Animal Coins - a reliable income stream for class upgrades.

Window Harlow: a hollow-eyed version of Dr. Harlow can be spotted through a window outside the hospital. The game has never confirmed what it means, but it is the most popular lore theory in the community.Off-map finds: players have reached out-of-bounds areas revealing an anomaly staging room, a broken baby-doll button with a loud jumpscare, and unused slime creatures. Treat these as developer leftovers, not intended content.Ghost rule: confirmed by several guides - never use a fire extinguisher on a ghost; it strengthens it. Taser or gun from behind works instead.Hiders: when a Hider chases you, jump on top of vending machines or stock shelves and wait for it to leave.
