S+ Tier

1 class

Secret Agent

S+

The strongest combat class in the game. Starts with a gun whose ammo regenerates over time, and the Scanner lights up red on any patient carrying anomaly symptoms.

Cost:
790 Robux (regional pricing varies)
Starter:
Gun (20 uses) and Scanner
Best for:
Players who want to hunt anomalies instead of only avoiding them
  • Bonus sanity and cash for killing anomalies
  • Highest Utility stat, so tasks, check-ins, and treatments complete much faster
  • Scanner reveals stacked anomalies that other detection methods miss

Premium Robux class. Very strong, but you can clear most content with coin classes first.

S Tier

2 classes

Surgeon

S

The most balanced class overall. Good at managing sanity and completing tasks, with speed and sanity boosts earned by healing patients.

Cost:
Animal Coins (Class Shop)
Starter:
Standard kit
Best for:
Experienced players who want an all-rounder
  • Speed and sanity boosts when you heal patients
  • Balanced Sanity, Utility, and Starter Money
  • Strong once you know the treatment flow

Requires coin grinding; best unlocked after you are comfortable with treatments.

Head Nurse

S

The best class for players who focus on healing patients. Extra inventory capacity means fewer trips around the hospital.

Cost:
Robux pass
Starter:
High starting money
Best for:
Team players who want to handle treatment duty
  • Increased inventory capacity
  • High starting money for early shop purchases
  • Efficient patient-focused play

Premium Robux class. Pick it if you mainly heal rather than fight.

A Tier

4 classes

Psychologist

A

Has the highest Sanity stat in the game and doubles every sanity effect - including the bad ones. Coffee gives 30 sanity instead of 15, but a wrong tase costs 30 instead of 15.

Cost:
Animal Coins (Class Shop)
Starter:
Standard kit
Best for:
Experienced players who rarely make mistakes
  • Doubles all sanity gains and losses
  • Highest maximum sanity pool
  • Never run out of sanity once your calls are reliable

Warning: the double penalty punishes wrong tases and missed anomalies heavily.

Doctor

A

Recovers sanity steadily by healing patients - 1 sanity per patient healed. Decent Utility stat, but low starting money.

Cost:
Animal Coins (Class Shop)
Starter:
Standard kit
Best for:
Players who treat a lot of patients each shift
  • +1 sanity per patient healed
  • Decent task speed
  • Low starting money, so early purchases are tight

Pair with the Head Nurse or a coffee routine for long runs.

Security

A

Starts with an X-Taser so you can deal with anomalies from the very first shift. Level 2 adds a Deployable Camera for checking patients remotely.

Cost:
1,250 Animal Coins
Starter:
X-Taser (5 uses)
Best for:
The best first paid class for most players
  • Starts with an X-Taser (5 uses)
  • Level 2: Deployable Camera for remote checks
  • Strong early-game anomaly control

Best value coin class - saves you from relying only on the free Shift 4 taser.

Firefighter

A

Added in the Hazmat Team update. Carries a personal Fire Extinguisher that recharges automatically and becomes fully immune to fire sanity drain at max level.

Cost:
1,100 Animal Coins
Starter:
Fire Extinguisher
Best for:
Surviving fire-heavy shifts and the slime event
  • Level 1: Start with a Fire Extinguisher and +1 inventory capacity
  • Level 2: Recover 1 sanity whenever you put out a fire
  • Level 3: Immune to sanity drain from fires and +10 maximum sanity

Situational, but excellent during the Hazmat event and late shifts with frequent fires.

B Tier

2 classes

Paramedic

B

A cheap, beginner-friendly class that boosts speed and carrying. Great for ambulance rushes where you need to move fast between patients.

Cost:
250 Animal Coins
Starter:
Large Speed Cola (6 uses)
Best for:
New players who want a low-cost upgrade from Intern
  • Starts with Large Speed Cola (6 uses)
  • Speed and carrying bonuses
  • Cheap upgrade path from Intern

One of the best first purchases - only 250 Animal Coins.

Secretary

B

Gains 1 sanity per check-in, rewarding you for working the front desk. The best beginner class for learning the check-in loop.

Cost:
Animal Coins (Class Shop)
Starter:
Standard kit
Best for:
Beginners learning to spot anomalies
  • +1 sanity per patient check-in
  • Rewards careful window and camera work
  • Simple, forgiving play style

A natural step up from Intern before moving to Psychologist.

C Tier

1 class

Nurse

C

A general-purpose class with no standout perk. Playable, but most players are better off saving coins for Security or Paramedic.

Cost:
Animal Coins (Class Shop)
Starter:
Standard kit
Best for:
Nothing in particular - a placeholder upgrade
  • Decent all-rounder stats
  • No major weaknesses
  • No major strengths either

Usually skippable. Go for Security or Paramedic instead.

D Tier

1 class

Intern

D

The starter class everyone gets. Starts with higher sanity than most classes, which buys you room to make mistakes during the first shifts.

Cost:
Free
Starter:
Standard kit
Best for:
The first 1-8 shifts while you learn the game
  • Higher starting sanity than most classes
  • Free - no unlock cost
  • Good for the tutorial shifts

Upgrade to Secretary or Paramedic as soon as you can afford it.

📈 Best Class Progression

If you are grinding Animal Coins, work through classes in this order:

  1. Intern (free) - learn the three-check routine.
  2. Paramedic (250 coins) or Secretary - cheap, forgiving upgrades.
  3. Psychologist - once your calls are reliable, the doubled sanity effects are a huge advantage.
  4. Doctor or Security - healing or anomaly control, whichever fits your play style.
  5. Surgeon - the best coin class once you know every treatment.

If you want to spend Robux, Secret Agent and Head Nurse are the two premium picks - combat power and healing power respectively.
