There is no "Doctor boss" in Animal Hospital. The real dangers are the monsters that shutters cannot keep out:
The Stalker: Follows you through the hospital. You cannot tase or shoot it - break line of sight and keep moving. Since the Harlow's Favorite update it can also check in as a patient with a messed-up application form; taking its photo makes it disappear.
Ghost: Spawns after an animal dies and is invisible until it attacks. Hit it with a taser or gun from behind. Never use a fire extinguisher on it - it strengthens the ghost.
Mimic: What a fake patient becomes once you let it inside. It eats other patients and attacks players, so the best defense is the three-check routine at the window.
Head Banger: Bangs its head at your office window. Give it a coffee and it stops; asking it to leave costs 20 sanity.
Tendrils: During surgery in Room 8, black tendrils burst from the patient. Complete the on-screen minigame quickly - never tase or shoot them, or the patient dies.
Bed Monster: Avoid the red zone under beds. Feed it maple syrup, an organ, or a transplant and it leaves.
